For the shared path line directives, all the credit goes to Jonathan. He did all the digging and assembling. I'm just the messenger.
As I read them, the guidelines he provided are from 2 sources: AASHTO: Guide for the Development of Bicycle Facilities, 1999 and MUTCD: Manual on Uniform Traffic Control Devices, 2003 Edition, both well-established documents so I'd say the "directives" are in existence.
